Now jdlover is domiciled in the USA and I did not perceive the need for a SIM while his offer was active; Murphies law has kicked in and now access to voice (and maybe, data) services for several weeks in the UK, France and Italy may be needed.

This is outside of Skype, Viber etc.

Does anyone have some up to date advice on the most cost effective options for this?

I might be able to sort you one out with a Giffgaff one if that's what you were after? Their prices look to be pretty decent, and speaking to cove who is using one right now it seems to be using the O2 network in the UK which is quite decent.
 
Don't use GoTalk's global sim. It didn't work the entire 4 weeks I was away due to their "network" issues. At least they refunded me the 300 i had pre paid with them

I believe Orange or Virgin in the UK have a decent one you can buy there...Italy..no brainer...TIM is terrific, easy to buy, great coverage, includes unlimited internet also as well as easy to recharge.
France was a nightmare...I ended up with one that had no internet included (no pre paids there other it...and cost a rip off)
