Have been investigating a number of apparent dead-ends.

Here's some information on how abusing a pre-paid card can be relatively easily tracked by the card program operator:

Have you ever looked at the balance statements of these cards on the net? It confirms that they are tracked in real time. Great article and thanks for posting.
I don't really mind. I've always worked on the assumption that this was happening.
