I was going to try DJ to the US in biz for the first time but decided to burn a pile of points to reduce costs a little and use QF instead.

I was massively overcharged for the fare due to an error, maxing out my credit card, which was rectified quickly after I called. I did write an email suggesting they compensate me for associated charges by the bank, which hasn't been answered (after 2 months)

Now only a few weeks before departure and two months after changing some dates QF have been unable to send me the revised itinerary, indeed it appears despite my itinerary being on my profile no actual ticket number exists yet...I have called about this three times.

Is this sort of thing common? I use a TA for non FF bookings although I know even TAs forget to issue tickets occasionally.

Edit- I don't usually ask for free upgrades but without a ticket number I can't get into the upgrade queue to F with the points amassed since booking....

At least you have a booking. If ticketed you'll see an e-ticket tab looking the booking up on www.classic.checkmytrip.com and you can print that (even if they can't send you a revised itinerary).

Also, if you are the only one in the booking, check here: LAN.com Imprima su ticket

Anecdotes have indicated in cases like this, the e-ticketing gets done eventually. But still, I'd keep on to it.

No they even told me it wasn't ticketed yesterday because the "taxes didn't match up". It's just frustrating having the same conversation with them three times running, and at the end still no ticket. I have arrived at the airport before with family and booking and no ticket number for a one month trip so am very sensitive to this (we were sorted out by the TA then and got on the plane)
