UA Co-pay for upgrades

Hi, 1st time posting here :) Hope you can help...

Does anyone know much about this co-pay system for upgrades on UA? Still the same number of points for non Y / B fares but now with a 'co-pay' fee of up to $500, AND discount fares can upgrade? My question is will M & H still be free to upgrade? And does anyone know what the fees are for each fare for AUS-USA flights?
