"We'd like to know what you think" survey problem

Yesterday I received the email from QFF Head Megan Whitlocke-Jones - "we'd appreciate your feedback .......................".

When I click on the link all I get is the error message "Unable to open survey".

Has anyone else encountered this? I tried several times with the same result.
