Tomorrow it’s on the road again heading north for another expedition to Dirk Hartog Island. I went last year, but the original plan for just PJM and me to go was upended by a string of circumstances. Rather than cancel, my brother and I went, with another expedition of six people this year...

That was one aspect of the three examples above that I wondered about. Were they so far off the mark that it should have reasonably triggered a "This doesn't look right." type of response? Or is it so finely balanced that it's not so easy to be attuned to that?

It appears that incorrect (manual?) data entry was the cause in each of the three cases and the error propagated undetected through the whole takeoff process. What processes are in place to either prevent that happening or detecting that something doesn't seem quite right before the point of no...
