Account hacked, or QF stuff-up?

I am unsure whether my account was hacked.
Recently when trying to get an award flight back from BKK, I discovered my name was changed. It now fails to match my passport.
I did not request a name change, and I did not receive notification from QF about any changes.
At this stage my points seem intact, but QF seems unwilling to provide evidence about how the change occurred, or to reverse the change. I need to protect my account and personal information (something QF seems unable to do).
Suggested next steps greatly appreciated.

Can understand a disinterested, poorly trained or previously burnt operative being reluctant 😟

Surely the process is to supply Qantas with an appropriately witnessed photocopy of your passport to the appropriate department and it will result in them rectifying it without much fuss.
