OK, the PDS uses the term "Hospital Compensation" and has a limit of $75 per day, see page 30. This is a payment for each day you are hospitalised and has nothing to do with authorised medical treatment in a hospital.I'd recheck that, I had believed the $8K was in relation to"expenses", not authorised medical treatment.
There is also a daily limit for these "expenses".

The cover under this section begins when you are hospitalised
overseas and ends when you are discharged from hospital.
What is covered?
We will cover you if you are hospitalised overseas for more
than 48 continuous hours because of an illness or injury which
first happened while you were outside Australia on your trip.

We will pay $75 for every 24 hours up to a maximum of $8,000.
Additionally, where an injury is as a result of a mugging attack
that has been reported to police we will pay you $500.

I noticed that the 250$ Activation still exists; make sense.
I am only using the card to make a Hertz reservation and not using it for other travel expenses.
The Hertz rental (Australia-Domestic) is 208$ for the week but then goes up to 270$ once location fee, taxes, admin recovery fee etc. are factored in.
The ANZ PDS says 250$ (inclusive of taxes). Am I reading it correctly that because I am paying 270$ (with the location fee, taxes, admin recovery fee etc.) factored in I am meeting the 250$ requirement?

I'm a little confused - are you asking if a car rental in Australia would activate the overseas travel insurance on the card? Because the answer would be "no".
To activate the travel insurance for a specific overseas trip, you need to pre-pay $250 or more of overseas travel expenses (e.g. flights, tours, hotels, etc) for that trip. Domestic travel does not count.
This is a really good benefit, methinks, little understood.The domestic rental car insurance does not need to be specifically activated. You don't even need to use the ANZ card to pay for the rental car.

In relation to this Credit Card; do i need to buy the 2nd item under optional i.e. Maximum Cover if I want to protect myself?
The only one I can 'skip' because the credit card covers is the 1. Accident excess reduction?

Hertz cars in Australia come with comprehensive insurance included in the base rental price (it is not possible to rent a car without comprehensive insurance in Australia). This insurance (note it's technically not insurance, it's a waiver, as you need a Financial Services licence to sell insurance in Australia) has an Accident Damage Excess (ADE) of ~ $4,000. Hertz also has a Single Vehicle Accident (SVA) Excess of $2,200 payable in addition to the ADE if you damage the car and no other is involved (e.g. If you hit a kangaroo). Accident Excess Reduction reduces the ADE to ~$500 and SVA is still applicable. Max cover eliminates both ADE and SVA.
ANZ Car Rental will cover ADE if you have an accident involving another car. It will not cover a cent if you have an accident not involving another car, which is the worst thing about it. I do not rely on it if renting from Hertz, only when renting from Avis, Budget and Europcar, as they do not have an additional SVA which would the ANZ car rental cover.
