I've been doing this for years with my wife who is in a wheelchair. Before she was confined to a wheelchair, it was like this:

You get off the plane last, or last in your class for international flights. If you can make it to the door, a wheelchair and attendant are waiting for you. If not, they have a special narrow wheelchair to take you to the door.

They wheel you to immigration, through the staff lane (almost no waiting), help you get your luggage, even getting another attendant if it needs pushing on a trolley. They help you through customs, and all the way to your transport.

Obviously, this needs to be booked in advance.

If you have a wheelchair, it is put in the hold on boarding, and returned to you on landing, and still you get an attendant.

All the way they will also manage your documents (landing card, passport etc) if required. These people are real troopers. Fell free to tell them of my appreciation over the decades.

I do this myself now, because I have a knee that plays up if stay sitting for too long. The walk through the airport and waiting in queues is not possible after 23 hours.
 
To add information shared in PM, this can be requested on booking, especially with a travel agent, and possibly on a web site. If not, you should ring the airline.

It can be booked on check-in, but assistance may not be available these days, or it may take longer at the airport. I've actually even done it in-flight when my knee swelled up flying economy (the last time for me!).
