Former VH-TJT crashes this morning

Not sure why QF is relevant to their reporting of the crash other than clickbait. Hasnt been in QF fleet for more than a decade
Is not terribly, just some local interest I guess - which I suppose is why the same aspect was mentioned in the OP in this thread.

Not sure why QF is relevant to their reporting of the crash other than clickbait.
For me it was only that QF used to own it, including the other one I posted. I don't know why some are so sensitive to that fact. And it is a fact that it was a "former"...
As I said, that it didn't happen on QF's watch says something...

pilot error
For every pilot "error" there are likely a whole sequence of events that lead up to it that are not pilot induced.

For some information on this, suggest reading/listening to a book/audiobook by Sidney Dekker: Drift into Failure. His other books/media are also very instructive as well

Your brain always has to get to anywhere in aviation, before the aircraft. You should always be working to a plan, not reacting to whatever has just happened. Getting behind the aircraft pretty well always leads to a mess (be it an approach, or anything else). What Juan is describing is pretty classic 'behind the jet'.
