Have you actually been successful?

I can tell you that they would have been successful. Coles/Woolies are different, but the payment provider is basically the same. The make big bucks either way.
As someone who worked for one the big 2 for a long time, it comes as no surprise.

So you haven’t used a Coles GC in Woolies or vice versa.

From the link provided in the opening post:

Where to use the Melbourne Central Gift Card ?

The Melbourne Central Gift Card can be used ...

I'm not convinced there can be any further clarity added by listing exclusions.
"Can be used" and "can only be used" are not the same. You are trying to convince everyone here that they are the same. I'm not sure your logic makes sense with the exclusions.

You missed this part below:

Standard Terms of Use (Givv Gift)
  • Redeemable anywhere EFTPOS is available.

A reasonable person reading the standard terms of use would interpret "anywhere" to be exactly as it states.

Has anyone had any experience with the FOS process? It says it's free so she might as well lodge a dispute - if anything the issuer would have to pay, unless anyone has other ideas.

Incorrect. You can’t read that statement in isolation of the broader context and other terms.

Is the issuer a member of FOS?

I’m still waiting for some to say it works. Why not? Sure. But does it? Looking like a big no.
In the case of the Melbourne Central gift card IT did work outside of the centre. so to answer your question I think its ok to use it since I am using my own funds. Not sure why you introduced coles and woolworths when we know that their gc can`t be used out of their store.

One can lodge a dispute with FOS only if one has lodged a dispute with issuer and not received redressal of their grievance there.

Because you were trying to do the same thing. Use the GC in a location that had nothing to do with the “business” that it was purchased from.

Not the same thing at all.

Coles and Woolworths are direct competitors. This is a card that is promoted for use at other stores that are independent of the business it was purchased from.

The only question is how wide is the scope - and the terms aren't definitive on that i.e. they are inclusive, but not exclusive.

We will agree to disagree on interpretation. As far as the T&C’s go, companies are inclusive of use rather than exclusive. Otherwise they’d have to list all retail places in Australia. Think about it.

No idea, it would be strange if an entity could set up a banking "service" and not be under FOS or similar.
Are you aware of any examples where this is not the case?

They arent the only shopping centre whose gift cards work elsewhere...

Gift cards, generally, have an exemption from a lot of the rules

This is about a gift card, not a banking service .

Actually you are not using your own funds - you used your funds to purchase a gift card. You are using a gift card - which comes with it the T&Cs attached to its use. You don't have the same T&Cs attached to your EFTPOS card or cash in hand.

I'm a bit surprised that no one has mentioned the fact that they have stored the OP's credit card number which is a strict no-no unless you as a customer authorise it.
