Late for redemption flight

Had a narrow squeak today. Got to airport with 5 mins to spare and had the horrible thought.
How would I go if I missed a redemption flight?
Anybody got any experience?
Would AMEX Platinum (that previously unbeatable card, with 1.5 for everything) insurance cover me? Not likely under their new terms!
How about the airline herself?
