New application for Signature Rewards - application crashing

Curious if anyone else has experienced this....
Last week I applied for a new NAB Signature Rewards card - decent sign-up bonus, on-going earn rate for KrisFlyer is pretty good, no foreign transaction fees and no monthly fee if you spend over $5K per month.
I completed the application online, was progressing well, and right at the final submit stage - received the message "Unexpected Error with your Application".
No worries I thought, NAB had sent me an email with a reference number earlier on in the application stage and I could access and resume the application. So I clicked on the link, had to start again - annoying but okay. Got 75% through and "Unexpected Error with your Application" again.
At this stage it was 11pm so I called NAB the next day.
The phone agent could see my application but there is an "error in there and I cant fix it. You need to do a new application over the phone, will take about one hour."
I asked the phone agent why - and he said "we have a new credit card application system and there are lots of issues, you arent the only person who has this problem".
The best part is - there is already a hit to my credit file and doing a new application will cause a new hit to my credit file. The phone agent said he can do a request to have the credit file corrected.
So - my question - has anyone else had this happen and does the adjustment to the credit file work as intended? Does the duplicate credit enquiry get removed?
And - this is terrible that they have known bugs in their software for new credit cards and they just keep using the broken application software....
 
I tried to fix a wrong record through ANZ; wasn't corrected. Gave up.
Btw I applied online for this NAB Sig about 6 weeks ago and no issues, in fact a very easy and quick application/approval (and since got the points, transferred to VA, and closed. Spend>$5k so fee free!).
 
I just got approved last week for a NAB Qantas rewards. Didn't crash but seemed a very short application process. Wasn't asked to upload any proof of income which I thought was odd because I've never had a NAB or Citi card before and don't bank with them. Not complaining but just thought it was strange.
