Does the form reporting non-compliant merchants still exist?

Palm Sugar Thai Cafe on Collins Street in Melbourne CBD has signs saying no Amex but its terminal actual takes it!

I have used it there too, but never got around to letting people know.
 
I saw that too at a cafe this morning. Paid by AMEX and felt "glad" for it. Though there was a time in the not too distant past that these costs were just a cost of doing business for the retailer rather than a stealth price rise.

Thought Debit was a flat rate much like EFTPOS - that's the way it was organised on a Suncorp Terminal I helped set up some years ago.
