Traffic is always solid in Manhattan itself, the bridge/ tunnel (depends which airport you're going into) into Manhattan can vary depending on time of day, road closures, things on etc
Also not sure when you land but I always get in early evening and it's a pretty smooth trip in.
I remember trying to get down 5th Avenue from the Upper East Side a few years ago for a show, Brooklyn Bridge was shut down for filming of Batman and traffic basically was at a standstill for a very long period.
As for the train (especially for first time visitors) - I wouldn't and I'm pretty familiar now with the subway etc. Last thing I feel like after about 23 hours of travel is lugging luggage on a train and then walking blocks with it!
