Do we know if this promo is still running? I have logged into the Amex site and got a quote for the insurance ($5.25 per month for $50,000), but nowhere can I see mention of the 20K points.

Make sure you've complied with their required format. An email is not sufficient to close your account according to them.
Eventually they will send you a confirmation of cancellation if you have complied with their requirements..
If this isn't working I'd send them a letter in the post. And I'd suggest you include something like the following:
'Should you ignore the cancellation notice already provided to you and attempt to debit my credit card for any future payments a charge of $150 shall be levied against you for each attempt.'
If this isn't working I'd send them a letter in the post. Tell them that the policy has already been cancelled and if they make any more charges to your credit card you will send them a bill.

It should but you may find that they inappropriately continue to allow debits to the original card for a few weeks. This will mean you'll need to argue with them to reverse the transactions.I like your style
