You can book the free flight in any name, whether they are a supplementary card holder or not.
 
Looking at the T&Cs link at the bottom of this page it appears that it may have been extended out to 12 months:

1. The 50,000 Qantas Points offer is only available to new American Express Card members who apply by 31 December 2013 via the online application form, are approved and make the required eligible Card spend(s). Card members who currently hold or who have previously held any other Card product offered by American Express Australia Limited in the preceding 12 month period are ineligible for this offer.
 
'a wee little while' being 6 months I believe, before Amex consider you a new customer.

Lets hope that the OP has a partner that can tag themselves in. Mind you - the card isn't worth it unless your spend is high on certain categories - I've kept the card because ours is and we have a few other alternatives in the wallet to use when it's not.

Ta for that your advice.

I have meals at restaurants twice a week and travel overseas for 4-6 weeks a year so the upgrade is probably worth it.
