Horses for course. Some find tapentadol better some find tramadol better. But overall we are seeing less SE with tapentadol
Pharmacologically Tapentadol better because it is direct acting unlike tramadol which is an inactive prodrug - it has to be metabolised into the active drug. As people have different abilities to metabolise tramadol into active ingredient, the inter individual variability in effect is greater in tramadol
Also tapentadol much safer in people with SSRI as it has minimal serotonin effects compared with tramadol.
But yes there are people who say they get SE with tapentadol but none with tramadol.
