There are plenty of people in the USA, that have AAdvantage Lifetime Platinum (Oneworld Sapphire) that have never been anything but a standard member or only a gold member.

Of course you don't even have to fly at all, to get 1MM or 2MM on AAdvantage.

There are a couple here in australia that are well on their way with not that much flying.
I'm aiming at my second million miles in 3 years with about a third from BIS.
