The supports are designed for one sleeper plus the bit in the ground. They aren't big enough for two (and it gets pricey).
 
I realise space may be limited, but I presume you have allowed for radial growth of the fruit trees as they try to branch out?

Or does she mean what the roots will do to the fence? Some of our trees have lifted the fence panel slightly, really noticed on one side of house where fence was installed 20years ago and the colourbond has a slight gap now.
 
The neighbouring orchard to ours had all their trees on espalier - unusual to see it on a commercial scale but they certainly looked impressive when in blossom! We would do a modified version on young trees to help influence the shape of the tree (ease of access for picking etc) but the wires would come down before maturity.

Espalier, as strictly understood, is not a typical orchard system for apples but, effectively, modern growing techniques commonly emulate it in various ways. Don't think it will be a problem. Again, had fruit trees all along the fence at the old house and no issues at all.
