When I used to smoke I refused to lightup in a restaurant and I always used to go outdoors for a cigarette. These days most restaurants and bistros are in separate rooms so you are away from cigarette smells.

Spend a few hours playing social darts, listening to a band, playing slot machines etc in a club and when I get home the cigarette stench on my clothes is overpowering.
 
I was surprised eating at a restaurant at Fed Sqare last year that they were allowing people to smoke while still serving meals. Admittedly we were seated in an "outdoor" area, but they had plastic screens in place for protection against the weather. 4 of the 5 other people I was with were smokers, so this suited them well, although they did wait until we had all finished eating before lighting up. I prefer the QLD approach of no smoking, even outdoors, in the immediate vicinity of people eating.

I don't have such an issue with smoking in clubs / pubs.
 
NZ approach is similar. If it has potential to be enclosed or partially enclosed it is deemed indoors and no smoking. Which means tables/benches that are streetside, harbourside and balconies are non smoking.

back to the off-topic topic of smoking in/at restaurants, tonight in Melbourne I ate at a restaurant at Southbank and sat "outside". There I was greeted with second hand smoke from many adjacent tables. It seems that it is no longer possible to sit outside and enjoy a smoke-free meal in Melbourne.
