From your quote of the terms
If you change or cancel your initial qualifying booking and/or travel is completed outside the promotional period, this bonus offer will not apply
My reading is that the "or" in the "and/or" removes the qualification of the flight with the changed dates.
It would be clear if the terms read:
If you cancel your initial qualifying booking OR change (the flight dates of) your initial qualifying booking and travel is completed outside the promotional period, this bonus offer will not apply
RedRoo - Time to clarify this PLEASE
To permit happy wandering

PS: the consensus on Flyertalk is:
I would expect that flight changes after 4 May would disqualify an originally qualifying flight.
Lets breakdown the term
1) If you (change or cancel) your initial qualifying booking and travel is completed outside the promotional period, or
2) If travel is completed outside promotional period - This second one is really a no brainer.
You don't get double status credits.
While the sentence construction could be improved. I do not see anything about changing an qualifying booking and travel is completed inside the promotional period.
